Club Officers

Starting Fall 2006 semester, all clubs will be using the same Participation Agreement form (aka Liability Form). 

All clubs should appoint an officer to be in charge of the Participation Agreement forms and the online roster.  All Participation Agreement forms must be entered online prior to submitting them to the Club Sports Office.

Participation Agreement forms need to be filled out for all Club Members and Coaches.  Advisors can fill out the Participation Agreement form if they too participate with the club.
